This notice describes how health information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Your rights

You may ask to inspect or obtain an electronic or paper copy of your health record; request a correction; request confidential communications; ask us to limit what we use or share; obtain a list of certain disclosures; obtain a paper copy of this notice; choose someone to act for you; and complain if you believe your privacy rights were violated. Some requests must be in writing and may be subject to limits permitted by law.

Your choices

For certain information, you may tell us your preferences about sharing with family, friends, or others involved in your care; disaster-relief situations; and communications about services. We will obtain written authorization before using or disclosing your information for purposes that require authorization, and you may revoke that authorization in writing as permitted by law. We do not sell your health information.

How we may use and disclose information

We may use or disclose health information to treat you, coordinate your care, obtain payment, and operate our practice. We may also disclose information when permitted or required for public health and safety, health oversight, workers’ compensation, law enforcement, judicial or administrative proceedings, organ and tissue donation, medical examiner or funeral director duties, research allowed by law, responding to lawsuits or legal actions, and compliance with other laws. We apply the minimum-necessary standard when it is required.

Our responsibilities

We are required by law to maintain the privacy and security of protected health information, provide this notice, follow the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when required. We may change this notice and apply the revised notice to information we already maintain. The current notice will be available at our office and on this website.

Email and website communications

Ordinary email may not be secure. Do not send symptoms, diagnoses, treatment details, insurance information, Social Security numbers, payment-card information, or urgent requests through the website appointment email. Call the office for sensitive or urgent matters. If you request unencrypted email communication after being advised of the risk, we will use reasonable safeguards and limit the information disclosed.
